Required for applicants entering grades Pre-K3 through 1:
- School tour and class visit
- Recommendation and Progress Report from previous school or day care (if applicable)
Required for applicants entering grades 2 through 3:
- Recommendation from previous teacher
- Most recent Report Card
Required for applicants entering grades 4 through 8:
- Recommendation from previous teacher
- Most Recent Report Card and Standardized Test results
- Entrance Exam (to be scheduled upon receipt of Recommendation and Report Card)
- Interview with Principal
All applicants will need the following:
- Birth certificate
- Immunization records
- Baptismal certificate and other Sacrament certificates (if applicable)
Tuition for the 2009-2010 School Year
- Annual rate for members of Catholic parishes
- PreK-K: $3,100
- Grades 1-8
- 1 child: $3,150
- 2 children: $5,190
- 3 children: $6,770
- 4 children: $8,250
- Annual rate for those who are not members of Catholic parishes
- PreK-K: $3,250
- Grades 1-8
- 1 child: $3,290
- 2 children: $5,440
- 3 children: $7,080
- 4 children: $8,630
General Fee:
- $275 per child
Mandatory Fundraising Goal* (based on family size):
- 1 child: $200 per year
- 2 children: $300 per year
- 3 children: $400 per year
- 4 children: $400 per year
*OLQA runs a number of fundraising events. This helps offset the difference between tuition payment and the actual cost of each child’s education, which exceeds $4,500 per year per student.

OLQA is open to students from 7:00am to 6:00pm
